La trágica pérdida de su hijo aún no nacido ha dejado destrozados a Kate (Vera Farmiga) y John (Peter Sarsgaard), lo que está pesando en su matrimonio y la frágil psique de Kate, que no para de tener pesadillas y se siente perseguida por los fantasmas de su pasado. En un esfuerzo por recuperar cierta normalidad en sus vidas, la pareja decide adoptar un niño. En el orfanato local, tanto John como Kate se sienten extrañamente atraídos por una niña llamada Esther (Isabelle Fuhrman).
Sin embargo, en cuanto Esther llega a su hogar, empiezan a producirse una serie de acontecimientos alarmantes que hacen que Kate llegue a creer que algo malo pasa con Esther, y que esa niña de aspecto totalmente angelical no es lo que aparenta. Preocupada por la seguridad de su familia, Kate trata de que John y los demás sean capaces de ver más allá de la dulce fachada de la niña. Pero nadie le hace caso hasta que puede que sea demasiado tarde... para todos.
